The CCBHC Care Coordinator will be responsible for providing enhanced coordination and care management activities for anyone at SIMHC who has been identified as needing care coordination services, per the CCBHC guidelines for care coordination. Care Coordination services may take place either: at SIMHC, in an outreach office, client home setting or other community organization setting.
This position will involve working collaboratively with staff members throughout SIMHC, as well as with community partners. This position is located in our SIMHC Fairfield outreach office and we would like to fill this position with someone that resides in Jefferson County. This position is an hourly paid position and reports directly to the Team Lead of Community & Outreach Services.
Responsibilities
- Documents care plans, case notes, client contact notes, and all other necessary documentation into the EMR.
- Assists in the screening, engagement and admission of clients into the CCBHC program.
- Assists in the use of client questionnaires as requested by SAMSHA and HHS
- Collects and tracks health status measures and data related to required Outcome Measures as requested by SAMSHA and HHS
- Provides care coordination by assisting clients in making needed medical or other healthcare appointments and tracking that follow up care did occur.
- Identifies and addresses gaps in care. Makes referrals to community and social support services as needed.
- Uses a recovery-oriented approach to assist patients in taking steps to improve their health and wellness.
- Provides support using a variety of approaches, which may include face-to-face visits, phone contacts and collateral contacts with the patient, the patient’s family or other providers on the person-centered and family-centered treatment plan.
- Plans, promotes and implements Wellness and Prevention activities for patients, including the development of a WRAP plan when identified.
- Assists in connecting individuals to community resources or services.
- Provide practical supports to individuals, such as transportation to appointments as needed and to ensure follow up on health-related concerns.
- Serves as a role model for recovery and promotes hope.
- Establish collaborative working relationships with other community stakeholder groups (for example: hospitals, primary care physician offices, health departments, HHS, etc.).
- Assist with scheduling appointments and setting up telehealth for patients.
Requirements

- Strong passion for improving the mental health care delivery system.
- Familiarity and proficiency with electronic medical record, electronic mail, and word processing required. Strong computer skills required.
- Hearing and visual ability which allows for necessary understanding while rendering professional and supervisory services.
- Physical ability to stand, walk and move for up to 7 hours in a working shift.
- Familiarity with community resources related to mental health is a plus.
- Bachelor’s Degree in a Human Services related field OR associate’s degree in a Human Services or closely related field is preferred but at minimum a high school degree or equivalent.
- Two years previous experience working with patients with a mental illness is required.
- Previous experience working with or for a community provider is preferred.
- Must carry current automobile insurance on the vehicle driven to and from work.
- Must have an active and valid Iowa Driver’s License.
- Must be able to pass an Iowa criminal background check.
- Must be able to pass Iowa HHS Adult & Child Abuse Registry verification.
- Must adhere to Driver policy 24.3 (4) as part of ongoing job requirement.
