Purpose
The Case Manager - Bilingual English/ Spanish will be working out of the Shannon West Homeless Youth Center under the direct supervision of the Program Director. This position provides direct case management services to homeless and at-risk youth in the Emergency Shelter Program.
Duties, Functions And Responsibilities
Work is performed in an indoor/outdoor environment with driving required at times. May be subject to sitting, lifting, bending, squatting, walking, standing, pushing and pulling, simple grasping and climbing.

Purpose
The Case Manager - Bilingual English/ Spanish will be working out of the Shannon West Homeless Youth Center under the direct supervision of the Program Director. This position provides direct case management services to homeless and at-risk youth in the Emergency Shelter Program.
Duties, Functions And Responsibilities
- Conduct initial interview, intake, and needs assessment for residents
- Complete any intake paperwork required
- Organize work schedule, and budget time.
- Create Client Centered Case Plan with residents within 72 hours of intake
- Monitor resident's progress and provide follow-up services by facilitating weekly sessions
- Complete monthly client centered case plan reviews with the client and obtain signatures
- Enter case notes in daily into HMIS and maintain accurate documentation of service
- Maintain audit ready files at all times
- Assist the youth in obtaining vital identification documents
- Complete exit summaries within 72 hours of exit from the program
- Organize, facilitate, and supervise daily groups/ residents and maintain logs as appropriate
- Attend various weekly meetings/ trainings
- Provides crisis intervention as needed
- Communicate effectively with business contacts from a variety of social, cultural, and economic backgrounds while maintaining clear boundaries
- Supervise residents prescribed and over the counter medications while maintaining medication document compliance
- Complete all required paperwork and logs, including incident reports, runaway reports, and resident tracking log
- Provide transportation to appointments, court hearings, or trainings when necessary
- Perform other duties as assigned by management for needs of the clients and/or HELP
- Knowledge of local social service agencies and programs as they pertain to homeless and at-risk youth
- Knowledge of applicable statutes, ordinances, and codes
- Knowledge of social service work, interviewing, and referral techniques
- Ability to follow complex instructions and procedures
- Ability to analyze situations accurately and adopt an effective course of action
- Ability to maintain order and exercise appropriate judgment in crisis situations
- Ability to work flexible hours as required by the resident case load
- Effective written and verbal communication to communicate with all business contacts in a timely manner
- General computer knowledge and ability to use Microsoft Office software
- Bachelor Degree in Social Work or related field, OR equivalent or combination of education and experience
- Non-expired Clark County Health Card
- Non-expired CPR certification
- Must be bilingual English and Spanish written and verbal
- Previous professional experience as required by education
- Must have a current NV driver's license and clean DMV record
- Experience and understanding of homeless at-risk youth issues
- Previous experience working in a similar setting
