What are the responsibilities and job description for the Patient Care Representative II position at Southern New England Healthcare?
SUMMARY OF RESPONSIBILITY:
The Patient Care Representative II for SoNE HEALTH Medical Group/Bear Pond Family Medicine and Pediatrics will provide support in the daily operations of the medical office and ensures that patients receive the highest level of care. This role ensures smooth operations in the medical office by handling patient appointments, phone calls, maintaining records, and creating a welcoming and organized environment. This position requires strong computer skills, attention to detail, familiarity with some medical terminology, and experience with health insurance.
This position is located in Enfield, CT.
ESSENTIAL FUNCTIONS:
- Schedule appointments ie. new patient appointments, sick visits, physicals, & office visits
- Answer telephone calls & process requests
- Process and file e-faxed documents into the appropriate patient’s chart
- Process incoming and outgoing medical records requests
- Scan paperwork into the appropriate patient’s chart
- Monitor patient portal messages & respond as appropriate
- E-fax referrals, school physical forms, & other patient paperwork to designated location
- Greet & check-in patients for appointments
- Verify patient demographic information ie. insurance, address, phone number, etc.
- Obtain all necessary signatures, insurance cards, & ID’s
- Collect co-pays & balances from patients
- Check patients out, schedule follow ups, & monitor when patients are due for a physical
- Review upcoming appointments to anticipate needs, supplies, & completeness
- Notify patients about labs due for their physical and/or medication check appointments, as appropriate
- Report needed supplies for ordering to Office Manager as inventory is used

QUALIFICATIONS AND COMPETENCIES:
- High school diploma or equivalent, Associate’s degree preferred
- Minimum of 2 years medical office experience or equivalent required
- Athena experience preferred
