About The Job
The Acute Care Case Manager evaluates the patient’s health status and the patient/family/significant other’s care capabilities to facilitate the discharge planning process. The case manager develops a multidisciplinary, culturally appropriate, age-specific discharge plan. They also complete clinical reviews utilizing Internal criteria for the appropriate level of care and insurance coverage.

What You’ll Do
- Assesses and identifies patients’ needs during the discharge planning process
- Provide resources and community referrals for continuity of care
- Care coordination with families, providers, and clinical team
- Utilization Reviews
- Bachelor's degree in a healthcare-related field or equivalent experience
- ASN/BSN preferred but not required
- 3 – 5 years of case management-related health care experience.
- A minimum of 7 years of experience is required without a bachelor’s degree
- Experience with patient documentation in an EMR system preferred
