Qualifications for Clinical Care Navigator:
- Primary series of the COVID-19 vaccine required
- Must have unencumbered RN/LPN License in New Hampshire.
- Must have 3 years of experience in LTC environment working with Medicare and Managed Care Services.
- Experience with the MDS/RAI process and/or case management preferred.
