Southern Tier AIDS Program, Inc. is seeking a full time Health Homes Plus Care Manager in our Binghamton office.  This position provides confidential outreach, intake, assessment and highly intensive care management services for designated individuals as part of the Medicaid Health Homes program. Individuals enrolled in Health Homes Plus are eligible for a higher level of support for up to 12 consecutive months.  

Salary:

  • $20.61/hour working 35-hours per week, may vary depending upon experience
  • Excellent benefits package including: life insurance, 401(k) retirement plan, medical and dental insurance, flexible spending plan and generous time off (24 days annually 11 paid holidays)
