Responsibilities
- Provide psychosocial support to patients and their families based on assessments of their needs and goals.
- Maintain accurate records of patient interactions and interventions.
- Collaborate with other healthcare professionals to develop and implement patient care plans.
- Utilize professional training and personal judgment to monitor the psychosocial assessment process.
Requirements
- Masters Degree in Social Work from a CSWE accredited School.
- Able to apply knowledge of special needs of hospice patients and families.
