Overview
We’re looking for a Licensed Practical Nurse (LPN) Continuous Care to join our team in Ontario, OH.
This position will report directly to the Executive Director/Administrator or Director of Clinical Services, but work under the direction of the Registered Nurse to provide continuous care visits to patients and families in a facility/unit or home setting. The Licensed Practical Nurse Continuous Care will actively monitor patients and report changes in their condition.
- Achieve palliation and management of acute medical symptoms during brief periods of crisis
- Under the direction of an RN, provide patient care services to hospice patients and supportive care to the patient’s family with a reasonable case load
- Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient
- Provide direct patient care, including administering medications and monitoring vital signs
- Provide education and support to patients and their families regarding their healthcare needs
- Report findings or changes in the patient’s condition to the Registered Nurse, interdisciplinary team and referring physician and document all reports
About You
- Graduate of an approved school of Practical Nursing
- A minimum of two (2) years’ experience as an LPN/LVN, either in long-term care, hospice, or home health.
- Three to five years’ experience preferred
- Compassionate, kind and empathetic to our patients and their families
- Ability to assess and respond to the needs of patients, families and caregivers
- Certified in CPR

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.
Salary Range: $20.25 - $23.85
