Job Description: Responsible for Responsible for HEDIS gap closures related to mental illness and substance abuse episodes associated with inpatient or emergency room visit.
ROLES AND RESPONSIBILITIES
- Under the direct supervision the BHS Manager, conduct 7-day and 30-day post discharge mental health assessments for adults and children 6 years of age and older, who were hospitalized for treatment of selected mental illness diagnoses or intentional self-harm diagnoses.
- Under the direct supervision the BHS Manager, conduct 7-day and 30-day mental health assessments for adults and children 6 years of age and older, after emergency department visit with a principal diagnosis of mental illness or intentional self-harm.
- Conduct mental health assessments for members ages from 6 to 99, who were assessed by mobile crisis or SASS and deflected from inpatient mental health and substance abuse treatment.
- Assist Health Plan Members with coordinating healthcare appointments to mental health, primary care providers.
- Provide appointment reminder calls to Health Plan Members.
- Educate Health Plan Members about community resources for managing their overall health.
- Link Health Plan Members to needed community and social services support.
- Complete accurate documentation of all assistance provided to Health Plan Members.
- Properly execute Health Plan Member referrals/concerns/grievances when applicable.
- Provide timely administrative notification of any critical Health Plan Member issues/concerns.
- Interact with staff and members to optimize workflow efficiency.
- Compliant with Performance Goals / Contract Deliverables.
- Help promote a company culture that encourages top performance and high morale.
QUALIFICATIONS AND EDUCATION REQUIREMENTS
- Master’s degree in social work, Sociology, Psychology, or related field.
- State Licensure of LPC or LSW.
- Minimum of 1 year experience with social work, mental health promotion, coaching, or wellness services.
- Minimum of 1-year mental health case manager experience.
- Knowledge of community health and social service agencies and community resources.
- Experience with electronic case note documentation.
- Strong customer service skills.
- Ability to work with others.
- Good problem-solving skills.
