SBH is seeking a qualified individual who will provide case management services to individuals and their families receiving services through the SBH operational programs. The case managers' activities involve admissions/intakes, preparation of service plans, coordinating and accessing service delivery programs, necessary support services, pre-admission and pre-discharge planning and casework documentation. The individual will be responsible for helping the client to maintain ongoing stability and to function at his/her highest level of productivity. Case management services include, but are not limited to: assessing, advocating, linking, coordinating, monitoring, problem-solving and transportation.
Responsibilities
- Completes client assessments and evaluations to determine the appropriate needs for services.
- Develops and completes client Individual Service Plans (ISP's) consistent with assessment evaluations.
- Counsels & assists the individual client in identifying, obtaining and effectively utilizing available community resources consistent with their identified needs in their ISP's.
- Provides follow-up services and monitoring activities to assure that community resources have been successfully accessed.
- Coordinates services with other provider agencies, organizations and individuals to assure that eligibility for benefits are in place and maintained concerning clients and their families.
- Provides/coordinates transportation services for clients & their families.
- Completes home visits with clients and their families to assure that ISP's are fully implemented and satisfactorily carried out as well as identify any additional concerns, problems or special needs requiring a response.
Qualifications
- Bachelor’s Degree in Human Services or related field with a minimum of one-year experience in working with individuals with intellectual disabilities and developmental disabilities.
- Experience in the provision and coordination of services to the intellectual and developmental disabled populations at a local community level (to include assessment, evaluation and planning of services, casework documentation, as well as utilization of available community resources for the mentally disabled) required.
- Knowledge of principles, techniques and practices relevant to case management, functional assessments, and their uses in service plans/coordination and general principles of case documentation.
- Knowledge of intellectual disabilities in adult, children & adolescent population.
- Must meet all criminal background and drug screening requirements.
- Valid driver’s license and safe driving record, required.
