About the Role
As a Family Practice Provider, you’ll deliver full-spectrum, patient-centered primary care to individuals and families, manage both acute and chronic health conditions, and collaborate with a multidisciplinary team to ensure every patient receives the highest quality of care.
Your Responsibilities Will Include:
· Delivering comprehensive primary care includes preventive screenings, chronic disease management, and acute care visits.
· Coordinating patient care plans with providers, nurses, social workers, and behavioral health specialists.
· Participating in rotating on-call duties for telephone triage.
· Accurately documenting encounters in Epic (EHR) within 48 hours.
· Completing required forms (e.g., FMLA, disability, worker’s comp) within two business days.
· Participating in peer reviews, quality assurance, staff meetings, and clinical committees.
· Supervising and mentoring medical students or residents as needed.
What We’re Looking For:
· Medical Degree (MD or DO) from an accredited institution.
· Board Certification (or Eligibility) in Family Medicine.
· Current Minnesota medical license and DEA certification.
· Passion for serving underserved, multicultural communities.
· Preferred: Community health experience and bilingual fluency in English/Spanish.
Position Details and Benefits
This is a full-time, salaried-exempt position.
• $180,000 - $250,000 Depending on experience.
• This position follows a 4-day workweek (4x10-hour days)—to promote healthy work-life balance. Includes 32 hours of patient contact time, averaging 16–20 patients per day in a supportive, team-based care model.

Work Environment
This position is based in a professional medical clinic setting and involves regular interaction with patients, colleagues, and healthcare staff. The role requires the use of standard office and medical equipment and may involve exposure to communicable illnesses, with strict adherence to infection control protocols.
Physical demands include extended periods of sitting or standing, frequent movement throughout the clinic, and the ability to assist patients with mobility. Providers must be able to lift up to 10 lbs, navigate stairs as needed, and possess strong communication and observational skills to ensure quality patient care.
