SUMMARY:
The Advanced Practice Provider (APP) performs and assists with all aspects of patient care including diagnosis, treatments and consultations. He/She may also conduct physical exams, order tests and serve as a patient’s primary healthcare provider. The APP functions under the direction of the Medical Director and/or his/her supervising physician as well as the clinic manager for non-clinical functions regarding the daily flow and management of the clinic. The APP is responsible for maintaining the health records of patients.
RESPONSIBILITIES:
* Performs complete medical history and physical exams on patients establishing differential diagnosis.
- Orders appropriate lab and diagnostic testing/procedures.
- Synthesizes date to determine primary diagnosis and therapeutic plans utilizing established medical principals.
- Demonstrates the ability to recognize and critically assess any changes in patient’s condition and reports such to the physician as needed.
- Acts as a liaison and facilitator between physician, staff members, case manager and attending physician regarding patient.
- Reviews results to verify all lab work, diagnostic results and other test results are available and notifies physician as appropriate.
- Facilitates coordination and development of a comprehensive and individual plan of care in collaboration with patient.
- Acts as a liaison with patient care staff.
- Dictates/documents patient information in appropriate format and following established guidelines.
- Maintains daily progress notes using established guidelines.
- Follows all established HIMS policies for maintenance and signing of medical records.
- Makes recommendations regarding clinic overflows and documentation requirements.
- Acts as lead for development of new workflows or implementation of new systems.
- Manages resources effectively.
- Ensures that patients are treated with respect and dignity and are processed through the clinic as quickly as possible while maintaining quality of medical care.
- Demonstrates dependability, accountability, and flexibility in all tasks.
- Works with clinic manager on regular ongoing basis to determine priorities for increasing patient satisfaction scores.
- Participates in Professional Development activities in addition to required CEUs.
- Participates with clinic manager in Practice Business Plan and bi-annual program evaluation.
- Consistently documents patient charts so charges can be rendered for services provided to patient. Charting completed according to medical staff guidelines.
- Keeps abreast of pertinent federal, and state regulations and laws and Tift Regional Health System, Inc. (“TRHS”) policies as they presently exist and as they change or are modified.
- Understands and adheres to: TRHS’ compliance standards as they appear in TRHS’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy; and HIPAA and TRHS policies regarding privacy and security of protected health information.
- Demonstrates the ability to perform tasks that meet the age-specific requirements of the persons, patients, vendors, and staff that the employee is charged to interact with as required by the position.
- Offers suggestions on ways to improve operations of department and reduce costs.
- Attends all mandatory education programs.
- Improves self-knowledge through voluntarily attending continuing education/certification classes.
- Maintains required competency levels as identified in written exams, skills checklists, skills labs, annual safety and health requirements as well as service excellence education hours requirements.
- Cross-trains in order to better assist co-workers and to provide maximum efficiency in the department.
- Volunteers/participates on hospital committees, functions, and department projects.
- Manages resources effectively.
- Reports equipment in need of repair in order to extend life of equipment and removes malfunctioning equipment out of service with timely reporting to the appropriate personnel.
- Makes good use of time so as to not create needless overtime.

OTHER INFORMATION:
Physician Assistant: Must be a graduate of a PA program. Must have current PA’s License. Two years of full-time clinical experience preferred. Active membership in American Association of Physician Assistants and Georgia Association of Physician Assistants preferred.
Nurse Practitioner: Master's of Science in Nursing required. Successful completion of a Nurse Practitioner program required. Three years of professional nursing experience in a clinic setting plus two years of Nurse Practitioner experience preferred. Must maintain current Georgia RN license. Nurse Practitioner certification is required.
