Responsibilities
Provide case management services to incarcerated adults that are in current need of mental health assessment or services . Services are provide primarily in the jail setting but could also be provided in the home, community, and outpatient settings. Assist individuals with securing social security benefits upon release, arranging housing/rental assistance upon release, access to other mental or regular health services as needed while incarcerated and upon release, coordinate appointments with other mental health professionals via telehealth as needed.
Summary
As a Care Coordinator for a Jail Setting, you will be essential in managing consumer care and services within a correctional facility. Reporting to the case management supervisor, your role involves conducting consumer assessments, ensuring compliance with HIPAA regulations, and facilitating discharge planning. Your core skills in managed care will be vital in helping the consumer access additional services upon release. By leveraging your relevant skills , you will contribute significantly to the health and well-being of the incarcerated population.
Qualifications
- Must possess a bachelor degree, preferably in a human service field.
- Experience working with individuals with mental illness, substance use issues, or other issues surrounding incarceration preferred.
- Must be able to coordinate schedules with jail staff, doctors, therapist, and other mental health professionals.
- Must have both verbal and written communication skills.
- Ability to handle crisis situations in the jail environment as needed.
- Proficient in dealing with individuals who could be resistant to interaction and suggested assistance.
Job Type: Full-time
Pay: From $16.83 per hour
