Care Coordinator for Jail Setting

Summary

As a Care Coordinator for a Jail Setting, you will be instrumental in managing patient care and ensuring compliance with HIPAA regulations. Reporting to the healthcare team leader, your core responsibilities will include patient assessment, discharge planning, and maintaining medical records. You will be vital in facilitating effective communication among healthcare providers. Additionally you will enhance the support provided to inmates, ensuring they receive comprehensive and compassionate care throughout their treatment journey. You will provide case management services to incarcerated adults with possible mental illness that are in current need of mental health services. Services are provide primarily in the jail setting but could also be provided in the home, community, and outpatient settings. Assist individuals with securing social security benefits upon release, arranging housing/rental assistance upon release, access to other mental or regular health services as needed upon release, coordinate appointments with other mental health professionals via telehealth as needed.

Qualifications

• Must possess bachelor degree
• Strong understanding of managed care and service provision
• Proficient in patient assessment and discharge planning
• Knowledge of HIPAA regulations
• Ability to remain calm in tense situation
• Work as a team with other individuals on the care team
• Proficient in securing resources from outside sources as needed

Job Type: Full-time

Pay: From $16.83 per hour
