As a Crisis Care Coordinator, you will be a vital part of our team, providing essential case management support to mentally ill individuals in crisis situations.
Responsibilities
A Crisis Care coordinator provides case management services to adults with severe mental illness that present in crisis in our outpatient office or community setting. You will assist individuals with securing social security benefits, food, housing/shelter/rental assistance, food stamp benefits, access to other health services, assistance with utilities, assistance in getting to and from appointments using the agency provided vehicle and any other basic living skill need that should arise. Services can be provided in the outpatient setting as well as the community, home, and other environments as needed. The care coordinator may also assist in patient transport to other psychiatric facilities as needed.

Qualifications
- Bachelor Degree required. Preferably in a human service related field
- Insurable driving record in order to assist with consumer transport to and from local inpatient facilities utilizing an agency vehicle.
- Ability to manage crisis situations in a calm and effective manner
- Ability to communicate both written and verbally
- Ability to work effectively with other office and team members for the betterment of the consumer is essential
- Strong office experience, preferably in a medical setting
- Familiarity with accessing local resources
- Excellent organizational and communication skills
- Ability to manage crisis situations effectively
- Previous experience with mental health consumers preferable
Job Type: Full-time
Pay: From $16.83 per hour
Expected hours: 40 per week
