- Position summary :
- Provides direct patient care to those individuals requiring health care in their homes, under the direction and supervision of the Registered Nurse. Works in a team relationship with other Home Health Services and contract associates and associates of Southwest General Health Center. Acts as a liaison among physicians, patients / caregivers, and co-workers. Participants in Health Center and departmental committees as assigned. Keeps current in nursing knowledge and required skills. Assists in the orientation process of new associates. The Home Health Licensed Practical Nurse must be able to demonstrate the knowledge and skills necessary to provide services appropriate to age groups according to specific chronological age, developmental age and / or psycho-social maturity.
- MINIMUM QUALIFICATIONS
- Education :
- Current licensure as a Practical Nurse in the State of Ohio. Advanced education in pharmacology required with ability to give medications.
- Required length and type of experience :
- Minimum one (1) year general medical surgical nursing experience required. One (1) year recent home care nursing experience preferred.
- Required licensure, certification or registry :
- Current licensure as a Practical Nurse in the State of Ohio. Must drive and have a valid Ohio driver's license. CPR certification required.Additional InformationSEN Learn More
