Position summary:

The Hospice Registered Nurse (RN) Case Manager provides both telephonic and direct care for an assigned caseload of patients in the Hospice Outpatient Program. The RN Case Manager uses the nursing process with care management strategies to coordinate care and provide nursing services for hospice patients and their families in the home, long term care facility, assisted living facility, or hospital.

MINIMUM QUALIFICATIONS

Education:
-
Graduate of an accredited school of nursing and current licensure by Ohio State Board of Nursing

Required length and type of experience:-
Previous experience in specific nursing area preferred.

Minimum of one (1) year experience as registered nurse in acute care/medical surgical area.

Prefer previous experience in hospice, palliative care or oncology.

Required licensure, certification or registry:-
Current license by the Ohio State Board of Nursing.

CPR certification required.
-
ACLS preferred.
-
If applicable, ACLS certification for Critical Care and Emergency Services required.

PALS required in Emergency Services.

Must have a valid driver's license and current insurance coverage.
-
Prefer national certification in hospice and palliative care or oncology.

CHPN certification is required within two (2) years of employment.
