Position summary:

Coordinate and staff patient care between various disciplines of the agency to ensure Start of Cares, Resumptions of Care, and visits are staffed in a timely manner while following regulatory staffing guidelines. Act as liaison between field clinicians and Assistant Clinical Manager. Collaborates with the manager and is responsible for participating in and using independent judgement in the hiring, supervising, training, performance management and evaluation of staff.

MINIMUM QUALIFICATIONS

Education:

Associate's degree required; graduate of an approved school of practical nursing or physical therapy.

Required length and type of experience:-
At least 1 year of home care experience required. Prior experience with scheduling preferred.

Required licensure, certification or registry:-
Must be a Licensed Practical Nurse of Licensed Physical Therapy Assistant in the State of Ohio and a graduate of an approved school of practical nursing or physical therapy.
