STAFF PHARMACIST – CARDIOVASCULAR SERVICES (primarily remote)
POSITION SUMMARY:
A professional caregiver, who prepares and dispenses medications, monitors for and promotes rational drug therapy. They act as a liaison between physician, patient and family, and collaborates with other health care members. The staff pharmacist must be able to demonstrate the knowledge and skills necessary to provide care appropriate to age groups according to specific chronological age, developmental age, and/or psycho-social maturity. Specific job duties may include:
- Patient education
- Obtaining prior authorizations
- Smoking cessation education
- Processing medication refills
- Medication reconciliation
- Partnering with physicians for high risk patients
MINIMUM QUALIFICATIONS:
Education: Graduate of accredited school of Pharmacy required.
Required length of experience: Previous hospital experience preferred.
Required licensure, certification or registry: Current licensure through the Ohio State Board of Pharmacy required.
