Job Summary: The Population Health Manager (PHM) works to improve the health of groups of people by identifying individuals within the group and developing targeted programs. The incumbent will work with a variety of stakeholders, coordinate population health activities encompassing the Health Care Model, Accountable Care Organization (ACO), and community health improvement plans. Their goal is to develop, implement, and evaluate population health programs and initiatives, increase the efficiency of healthcare delivery, and reduce costs while also improving the patient’s experience.
- Bachelor’s degree in public health, Healthcare Administration, Nursing, Business Administration, or a related field.
- Experience in healthcare management, population health, or value-based care programs.
- Strong knowledge of healthcare quality metrics, data analytics, and care coordination models.
- Experience with electronic health records (EHR) and data reporting tools.
- Excellent communication, problem-solving, and organizational skills.
