Following the mission and vision of SWMTCHC, and under the direction of the Director of Behavioral Health and Lead Care Manager, the Care Manager is responsible for assessing every patient in the identified panel for Social Drivers of Care and providing patients with referrals and access to the identified needs to support the patient, their primary care team, and improve overall health outcomes. This is a high-paced environment that requires compassion for others and a desire to make a difference in the lives of our patients through developing a trusting, helping relationship as the primary point of contact for the patient panel.
Position Requirements
Education: Degree in social work, public health, human services, or related field preferred. Experience in these fields may be considered in lieu of education.
Experience: Prior experience in a healthcare setting or care/case management role preferred. Experience preferred serving a diverse population including those who are vulnerable and/or may have high needs.

- Serves as the primary point of contact for individuals in the identified patient panel
- Maintain regular contact with patients according to their needs, communicate patient needs to the care team, and document appropriately in the medical record within established timelines
- Outreach patients according to healthcare needs
- Assist with outside and internal referrals to services
- Develop and maintain positive relationships with patients
- Assess barriers to patient care using Social Drivers of Health screening tool and provide services to address barriers to improve health and wellbeing of the patient population
- Schedule appointments, assist with paperwork, and make referrals both internally and externally for patients
- Within the Care Management Department, there are several specialty areas that could be assigned such as: Lead, Healthcare for the Homeless, Pediatrics, Mobile Van, etc.
- Ability to embrace and learn new technology
- Ability to become proficient in the use of clinic-specific software programs including EPIC
- Ability to assess importance and prioritize tasks
- Basic understanding of Motivational Interviewing and the importance of addressing Social Drivers of Health

Physical Demands/Working Conditions
General office/clinic conditions are pleasant; good, clean working conditions where accidents and hazards are negligible; requires short periods of moderate lifting, pushing, or pulling objects up to twenty pounds. Clear diction and acute hearing are necessary for effective communication with the staff and public.
