We are currently seeking a Licensed Practical Nurse (LPN) Care Coordinator to join our team in Tazewell, VA.
Position Summary:
The LPN Care Coordinator plays a crucial role in coordinating patient care, ensuring continuity of care, and improving health outcomes. This position will support patients by facilitating communication between providers, coordinating referrals, monitoring chronic disease management, and promoting preventative care. The LPN Care Coordinator will work closely with a multidisciplinary team to ensure patients receive the care and resources they need.
Key Responsibilities:
- Coordinate patient care plans, ensuring timely follow-ups and adherence to treatment plans.
- Serve as a liaison between patients, healthcare providers, and community resources.
- Conduct patient education and counseling on chronic disease management, medication adherence, and lifestyle modifications.
- Assist with care transitions, including hospital discharges, to reduce readmissions and improve patient outcomes.
- Monitor and track patient progress, ensuring timely interventions as needed.
- Facilitate referrals to specialists and other healthcare services, ensuring efficient communication and follow-through.
- Support quality improvement initiatives and participate in team-based care strategies.
- Document all patient interactions accurately and timely in the electronic health record (EHR).
- Participate in case management meetings and interdisciplinary team discussions.
Qualifications:
- Current and unrestricted LPN license in the state of Virginia.
- Minimum of 2 years of clinical experience, preferably in a primary care, community health, or care coordination setting.
- Experience with chronic disease management, patient education, and care coordination preferred.
- Knowledge of FQHC operations, value-based care models, and population health management is a plus.
- Strong interpersonal, communication, and problem-solving skills.
- Proficiency in electronic health records (EHR) and ability to document patient care effectively.
- Ability to work collaboratively within a multidisciplinary team.
