Southwestern Behavioral Healthcare is currently seeking a full-time Care Coordinator to add to our team of professionals. This position will provide care coordination support by facilitating screening tools, case management services, and care planning services to clients and their families. The Care Coordinator will provide skills training and treatment planning in office or in the community as needed.

WHAT IS THIS POSITION RESPONSIBLE FOR?
- Conduct screening tools as a part of the assessment process that will be used by treatment team for further evaluation and review.
- Provide phone, Zoom, and in person interviews with community members, or their families, to screen for mental health and substance use disorder needs and safety concerns.
- Services are delivered utilizing a recovery/person centered philosophy and orientation, keeping with the preferences of the person receiving services and their care needs.
- Confer with related health professions in relation to the need for interdisciplinary information sharing and warm hand off.
- Provide Care Coordination through identification of needs and barriers and assist with accessing resources. Coordinate with other systems to meet the needs of those served, including criminal and juvenile justice and child welfare.
- Monitor client’s use of services, ongoing safety, and successful engagement with referrals.
- Participate in relevant staff training and daily treatment team meetings.
- Collaborate with the team to assure ongoing needs of client are being addressed.
- Maintain appropriate records of services to reflect assessment of risk, safety planning, engagement, psychiatric advanced directives, and care coordination activities.
- Coordinate access to continuum of care for individuals making contact with Crisis Services.
- Serve as Insurance Navigator for individuals seeking assistance with enrolling in an insurance program.
- Assist the crisis services team with developing and accessing health interventions.
- Assist those with limited English proficiency, or other communication challenges, in accessing services via appropriate methods.
- Assist consumers with implementation of crisis prevention plans to prevent and de-escalate future crisis situations.
- Assist consumers with access to CCBHC services regardless of area of residence.
OTHER FUNCTIONS:
- Maintains working knowledge of community resources and entitlement programs.
- Engage clients who have recently accessed the local Emergency Departments for a Mental Health or Substance Use related crisis.
- Utilize electronic medical record for clinical documentation.
- Provide data tracking for purpose of outcomes monitoring.
Job Qualifications
WHAT'S REQUIRED FOR THIS POSITION?
- Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree required.
- Must be at least 21 years of age and maintain a valid driver’s license.
