WHAT IS THIS POSITION RESPONSIBLE FOR?

  • Coordinate services primarily for adults seeking substance use disorder and co-occurring care from Southwestern.
  • Assist with removing barriers to accessing services for clients andfamilies.
  • Conduct pre-intakes and assist with caseload management in order to reduce administrative burden for therapists and prescribers.
  • Provide office and community-based skills training and development including parenting skills.
  • Provide case management services forbased on needs. Case management includes but it not limited to communicating with all parties involved in the treatment, identifying treatment needs, and facilitating all necessary referrals.
  • Maintain appropriate records of services to reflect patient progress/needs; develop care plans and goals in conjunction with clients and referral sources.
  • Maintain documents, collect data, and submit reports according to established time frames.
  • Attend and participate in regular staff meetings.
  • Participate in ongoing staff training and development activities as assigned.
  • Provide emergency counseling and crisis intervention when necessary.
  • Conduct consultation and education activities to community and professional groups asneeded.
  • Maintain contact with other service providers, community agencies, schools, and civicorganizations to enhance community awareness and public relations.

MINIMUM QUALIFICATIONS:

  • Bachelors degree in Social Work, Psychology, Sociology, Criminal Justice or other relateddegree required.
  • Experience working with adults with a substance use disorder is preferable.
  • Possess the ability to assess emergency situations and react in an appropriate andtherapeutic manner.
  • Must possess a valid drivers license.
