Position Overview:

Southwestern University has an immediate opening for a Case Manager and Outreach Specialist. Reporting to the Director of Counseling & Health Center. The Case Manager and Outreach Specialist develops a program to address the needs of students struggling with issues related to student success, such as mental and emotional health, academic progress, and social adjustment. Collaborates with various resources on and off campus, makes referrals as appropriate, assists with coordination of care, and assists in the development of re-entry plans for some students as they return to campus. Coordinates with family members as appropriate. Works with students requiring high levels of care and contact. Such students will generally be identified through the Care Team. They will also conduct training and outreach initiatives that focus on primary prevention and health promotion to the campus community in public health topics such as mental health awareness, alcohol, and other drug education, sexual assault risk reduction, eating disorder prevention, suicide prevention, systemic health disparities related to marginalized populations and other issues relevant to the community. This position is funded for two (2) years, with the possibility of renewal. This is a full-time, 11-month, exempt position eligible for our comprehensive benefits program.


Essential Duties:

  • Proactively build and sustain relationships with students identified as high risk or a part of a marginalized population; Build a caseload from this student population and meet as needed throughout the academic year.
  • Identify obstacles, both internal and external, that hinder student success and retention; develop metrics and assessment strategies to evaluate targeted intervention initiatives.
  • Attend Care Team meetings and coordinate support initiatives targeting systemic health disparities among SU’s marginalized or underrepresented students.
  • Coordinate collaboration on health initiatives that directly correlate with obstacles to students success with local community stakeholders and services.
  • Coordinate and collaborate on holistic health promotion and retention projects with student organizations, relevant staff departments, and faculty interested in supporting these projects.
  • Using evidence-based practices, primary prevention, and environmental and behavioral change strategies, create holistic health initiatives related to public health topics.
  • Be responsive to the University’s crisis hotline (rotating on-call) and provide crisis intervention.
  • Complete contracts for presenters engaged by counseling, health, and relevant committees and ensure transportation, lodging, and the like are secured.
  • Maintain all financial documents around resources used in promotion and outreach activities.
  • Select, train, and supervise Master’s degree seeking practicum students who can:
    • Support case management initiatives by maintaining a select case load and meeting with students in a manner similar to the professional staff, as appropriate
    • Create, provide, and coordinate population-specific initiatives, interventions, risk management training, social marketing campaigns, and other public health interventions.
    • Plan, market, implement, and evaluate health promotion programs for SU students that have a high likelihood of achieving measurable improvements in health behaviors and/or academic status.
    • Attend relevant university committee meetings to implement proposed initiatives.

Additional Duties

  • Stay abreast of current developments in the field.
  • Select, train, and supervise student interns and/or student workers, from the Southwestern student population, who will assist the Health Educator by providing peer-to-peer education and outreach.
  • Perform other duties as assigned

Minimum Qualifications:

  • Master’s degree or equivalent in Higher Education with a Counseling Track, or in Social Work, Psychology, Counseling, Mental or Behavioral Health, or related field.
  • One (1) year experience in a case management-related position.
  • Demonstrated ability to work effectively with students and student organizations.
  • Knowledge of diversity as it relates to case management, public health topics, and system level initiatives.
  • Ability to work both independently and as a member of a team.
  • Excellent computer skills utilizing software such as Microsoft Word, Excel, and Google’s suite of word processing and data management products.
  • Demonstrated leadership skills, including a strong work ethic.
  • Willingness to adhere to the University’s core values.
  • Possess excellent interaction and communication skills (verbal and written), including outstanding diplomacy skills.
  • Ability to be flexible, adjust to fluctuating priorities and produce reliable work product.
  • Demonstrated professional manner with all internal and external University constituencies.
  • Proven ability to perform well in stressful situations.
  • Understanding and commitment to a liberal arts education, including the ability to relate to a liberal arts faculty.
  • Ability to attend work consistently with a willingness to work a flexible schedule, including evening and weekend hours as needed.

Preferred Qualifications:

  • Licensed Social Worker or Licensed Professional Counselor.
  • Hold a Community Health Worker (CHW) or a Health Education Specialist (CHES) certification
  • Three or more years of related experience.

Starting annual salary: $60,000. The final offer for the successful candidate will be commensurate with experience and may exceed the starting salary.
