What are the responsibilities and job description for the Centralized Medication Refill LPN or Certified Medical Assistant - Putnam Medical Group position at Southwestern Vermont Health Care?
The Centralized Medication Refill Licensed Practical Nurse (LPN) or Certified Medical Assistant works with clinic staff, clinicians, triage RNs, pharmacies, and care teams to process electronic, faxed, and patient portal medication refill requests, electronic medication renewal requests, and medication prior authorizations. They will work with clinical staff remotely and will follow protocols for refilling prescribed medications, as well as prepping laboratory orders and requesting patient’s appointments using the electronic medical record (EMR). The Centralized Medication Refill Nurse/CMA will also participate in obtaining prior-authorizations for medications using electronic platforms as well speaking on the phone to insurance companies and pharmacies. The Centralized Medication Refill LPNs and CMA's will participate in ongoing quality committees, projects and goals for ambulatory patient care in coordination with the Centralized Nursing Services team and will engage in patient education and clinic operational activities as needed in support of the provision of care to patients.
Requirements: Graduation from an accredited school of nursing required. Prior clinical nursing experience preferred. Ability to obtain a valid Vermont State LPN license or Certified Medical Assistant by date of employment required. Health Care Provider BLS certification required within 90 days of hire.
