Overview
PURPOSE STATEMENT:
Provide treatment services to patients and their families with social, personal, and/or occupational problems associated with mental, emotional, and behavioral disorders and/or substance abuse/dependency within the framework of a treatment team concept.
Responsibilities
ESSENTIAL FUNCTIONS:

Assess psychosocial aspects of patients and/or significant others regarding personal, social, emotional, spiritual, and chemical dependency history utilizing the knowledge and skills related to the developmental stages of adulthood, adolescence, and the special needs of the geriatric population.
· Develop Interdisciplinary Treatment Plan and Treatment Plan Update in conjunction with other members of the treatment team, based on the assessed needs of the patient.
· Record Psychosocial Assessment, Interdisciplinary Treatment Plan, and Treatment Plan Updates in the Medical Record within the designated time frame.
· Demonstrate knowledge of psychiatric terms used to evaluate mental status and describe abnormal findings.
· Participate in treatment team meetings and makes recommendations regarding treatment issues and discharge planning.
· Provide documentation indicating progress, or lack of progress, related to the goals of the Interdisciplinary Treatment Plan.
· Assist the treatment team by developing discharge and aftercare plans for patients with complex needs and/or placement issues.
· Serve as the coordinator for patients involuntary committed, providing communication back and forth with court, ensuring completion of all necessary paperwork as well as appearances in court.
· Maintain ongoing contact with referring individuals and/or agencies as well as a comprehensive listing of resources for aftercare.
· Care for the patient/family and other staff members of the healthcare team, utilizing a customer service approach.
· Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
OTHER FUNCTIONS:
· Perform other functions and tasks as assigned.
Qualifications
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

Bachelor’s degree in Counseling, Psychology or related field required.
· One or more years’ of previous experience in an inpatient Behavioral Health setting and/or previous experience with substance dependence/mental health treatment required.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
· Requisite state or national licensure for professional therapy providers or eligible for state licensure as a professional therapy provider.
· CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).
· First aid may be required based on state or facility.
