Overview:
Under direction of ADRC Program Manager, performs work of considerable difficulty in the professional assessment, screening and placement in community-based services for older adults and people with disabilities. This work is performed in an environment equivalent to a call center setting. Also, will participate in program development, provision of information and assistance, coordination of area-wide information on services available to older adult community, and assist on the Transporation queue on specified scheduled days and as back-up for the Transporation Coordinator as necessary.
Essential Functions:
- Perform intake, assessment, screening, and placement via telephone and occasionally in person for older adults and people with disabilities in aging services programs in the 14-county Southwest Georgia region.
- Provide information and assistance to senior citizens and their families.
- Maintain essential / confidential records.
- Identify needs and desired services as stated by referral source or applicant.
- Provide technical assistance to aging services contractors for new and existing aging services programs.
- Assist with statistical data compilation.
- Conduct quarterly assessments of waiting list(s) for aging services to be provided to frail and/or disabled older adults, i.e., home-delivered meals, homemaker services, adult day care, respite care, and the Elderly Disabled Waiver Program (EDWP) (formerly CCSP).
- Assist with advocacy on behalf of older residents of Southwest Georgia.
- Fulfill a customer service role, ensuring that the consumer experiences a welcoming atmosphere and consumer satisfaction is achieved.
- Log all contacts and referrals into Harmony database.
- Exhibit the technology skills related to the required work (word processing, spreadsheets, database, internet research, mail, email, phones, copiers, etc.).
- Establish and maintain effective working relationships with clients, families, caregivers, service providers, volunteers, community agencies and the public.
- Interview clients, consumers, and family members to evaluate situations presented and determine the extent and need of services and supports.
- Perform Options Counseling utilizing a person-centered approach in providing services to individuals.
- Perform other duties as directed.

Qualifications:
Knowledge: Aging services programs to include transportation, housing, insurance counseling, legal assistance, nutrition programs, and community-based in-home services. Also, must be familiar with computer software, survey and/or other information-gathering techniques, and database manipulation.
Education: Minimum of Associate degree is required. Degree in Nursing or Social Work preferred. Degree in Gerontology, Health Sciences, or related field acceptable.
Certification: After completion of one year of service, Certified Information and Referral Specialist – Aging (CIRS-A) must be obtained. This certification is through the Alliance of Information and Referral Systems and is a requirement of the position. Must also obtain Options Counseling certification with Division of Aging Services.
