Facility: Bedrock Rehabilitation and Nursing Center at Island
Location: Merritt Island, FL
Position Type: Full-Time
Salary Range: $36 - $42 per hour
Are you an experienced RN with a passion for enhancing resident care? We are seeking a dedicated MDS Coordinator to join our team and ensure excellence in care planning and compliance.
Key Responsibilities:
Evaluate and Enhance Care: Oversee the quality of care for long-term residents, ensuring their needs are met with excellence.
Comprehensive Assessments: Conduct initial interviews and thorough assessments for new residents.
Care Planning: Develop detailed care plans focusing on short-term and long-term goals, tailored to each resident's needs.
Collaborative Leadership: Facilitate meetings with multidisciplinary teams, caregivers, and families to align on care objectives.
Community Collaboration: Partner with community specialists to achieve the best outcomes for residents.
Team Integration: Participate in nursing management meetings, ensuring effective communication and collaboration at all levels.
Education and Training: Lead in-service education sessions on MDS, RAPS, and Triggers, promoting knowledge and best practices.
Compliance: Adhere to the MDS/Care Planning schedule, particularly for non-Medicare residents.
Qualifications:
Florida RN License: Must hold a valid RN license in the state of Florida.
Experience: At least 2 years of experience in a Skilled Nursing Facility (SNF).
Expertise: Strong knowledge of MDS, Medicare, and the care-planning process.
