Position Summary

The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines and standards of NFP and of the lead agency. Nurse home visitors report to the NFP Nurse Supervisor/Manager.
Minimum Requirements
- Education BSN required
- Experience Two years of experience in maternal /child, public health, home visiting, mental/behavioral health preferred.
Required Licenses/Registrations/Certifications
- Current RN license in good standing
- BSN required
- BLS
- CLC or IBCLC preferred
- Two years experience in maternal/child, public health or mental/behavioral health preferred.
Core Job Responsibilities
- Completes all required NFP and hospital education.
- Provides home visits to women and families eligible for NFP.
- Carries a minimum caseload of 25 clients.
- Adheres to nursing process and the NFP model of home visitation.
- As requested/required participates in community outreach and supports public awareness of NFP
- Assesses physical, emotional, social, and environmental needs of women and their families as they relate to NFP domains.
- Provides education, support, and referral resources in assisting women and their families in attaining targeted goals.
- Maintains confidentiality
- Maintains clear, effective, open and honest communication with both internal and external customers and colleagues.
- Complies with established policies and procedures.
- Follows established documenting policies and procedures.
- Valid driver’s license and insured automobile required.
- Able to travel by air to Denver, CO for all face to face education.
- Must be able to work flexible schedule including some evenings and weekends.
- Participates in bi-weekly meetings and case conference
- Participates in weekly meetings with Supervisor/Manager
Utilizes basic computer skills
