POSITION SUMMARY
Position will provide professional medical services to patients in a clinic setting under the direction of a licensed physician, advanced nurse practitioner, or physician assistant. Position will assist in training of other Agency staff regarding blood borne pathogens, tuberculosis, first aid, and HIV Workplace Guidelines. Provides education and medical services for people with HIV disease and other healthcare conditions. Assists in providing medical case management services. Position provides technical assistance regarding tuberculosis activities. This position will assist the medical case manager and other senior staff in developing clinical services and accessing other funding sources.
ESSENTIAL RESPONSIBILITIES:
- Vaccine Coordinator for the Clinic
- Monitors clients, following protocol as outlined in the standards of care
- Provides medication adherence education and appropriate interventions to each client as needed.
- Assists medical staff with all procedures and follows directives
- Screens clients on clinic days, obtaining weight, vital signs and history
- Draws labs, do UAs, check blood sugars
- Logs and reports lab results
- Educates client regarding medications, adherence, safe sex and preventive health management
- Screens medical related walk-ins for vital signs and severity, refers emergencies to ER
- Schedules clients to do routine labs and procedures ordered by physician
- Reviews and reports panic labs to medical supervisor as received
- Screens medical related phone calls, assessing client needs
- Administers vaccinations, TB tests, and injections as ordered
- Tracks clients TB testing and vaccinations for compliance to agency protocol
- Updates and documents accurately in client charts
- Communicates with case managers and other staff to ensure quality client care
- Calls in prescriptions and refills to pharmacy
- Performs patient follows ups and makes referrals as needed
- Follows agency protocol when handling biohazardous and infectious materials
- Orders supplies for clinic as needed
- Fills out state medical applications and medical certification forms
- Completes and processes patient assistance forms for medications
- Codes and completes billing forms according to funding source requirements
- Prepares confidential and non-confidential correspondence in a professional style and format
- Prepares time reports and mileage accurately and on time
- Complies with all HIPAA requirements
- Conducts community presentations as necessary
- Attends all staff meetings and trainings as assigned by supervisor.
- Conducts self in a manner consistent with professional standards.
- Adheres to all Agency personnel and program policies
- Other related duties as assigned
Qualifications
QUALIFICATIONS:
- Licensed vocational nurse by the state of Texas or Licensed Practical Nurse by the State.
- Outpatient Clinic experience preferred.
- Insurable for professional liability.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
- Good independent judgment skills.
- Excellent written and oral communication skills.
- Able to schedule work activities according to priority.
- Ability to complete documentation in legible and professional manner
- Needs to be familiar with community resources and be able to direct persons to the appropriate services.
- Ability to follow procedures while working in an independent environment.
- Must be self-starter and self-directive.
