Title: Client Care Coordinator Division: SILVER
FLSA:Non-Exempt Supervisor: Clinical Supervisor
Pay Range or Rate: $35-$38 per hour Commensurate on experience. May be eligible for hiring bonus.

Summary
Under the direction of the clinical supervisor and Division Director, the CCC has primary clinical responsibilities in but not limited to the following areas:
Essential Functions
Provide psychiatric evaluation to older adults and adults with disabilities who are frail, functionally impaired and/or mentally ill including assessments, psychosocial intakes, emergency/crisis evaluations.
Provide psychotherapeutic services to individuals and groups including care plan services.
Provide case management services to ensure a continuous system of care to the targeted population.
Participate in the inter-disciplinary team in the development of individual treatment and discharge plans for clients.
Provide clinical consultation to agencies and constituents of the community as appropriate.
Participate in utilization review and other quality assurance activities.
Maintain appropriate and thorough documentation of client records.
As needed, provide 24/7 crisis intervention support.
Conduct regular community education and promotional outreach.
Participate in regularly scheduled clinical supervision (group & individual) provided by a licensed clinical staff.
Responsible for communicating effectively (verbal & written) with clients, family members, service providers, co-workers, adult protective services workers, local, state and federal departments.
Ability to drive own vehicle and navigate throughout communities of Los Angeles County.
Coordinate with outpatient clinics, and various publics and private community health, mental health, and social services agencies to ensure quality care services.
Other Duties:
Attending/presenting in community meetings, trainings, and/or conferences as related to projects goals.
Maintain relationships and networks with other service providers and organizations that serve the program’s targeted population.
Some long distance and out-of-state travel required.
Work on other appropriate tasks as assigned by the supervisor.
Secondary Functions
Perform in assistance as a Preceptor for accredited university programs (i.e. BSW and/or MSW).
Minimum Qualifications - Knowledge, Skills And Abilities Required
Registered with CA BBS as ASW or MFT trainee
At least 2 years of experience in health, human services, case management, gerontology or related fields.
Ability to perform psychosocial clinical assessment.
Displayed ability to work effectively with people, particularly with culturally/ethnically diverse communities.
Ability to effectively use word-processing, spreadsheet, PowerPoint and database programs.
Ability to work independently and within a team structure.
Able to work nontraditional hours and weekends.
Strong understanding and working knowledge with ethnic minority communities.
Passion for serving older adults and to increase healthy aging.
Have a working knowledge of local (city and county) departmental protocol.
Have own transportation with valid California Driver’s License and automobile insurance.
Must be or in the process of registration with the Board of Behavioral Sciences in California.
Bilingual Skills, Spanish
Some experience and/or understanding of Department of Mental Health protocols.

Environmental Conditions (Working Conditions)
Must be able to work in the community (field) based settings. Requires driving in all weather conditions as well as challenging neighborhoods throughout Los Angeles County. Conditions may also involve working with challenging clients in the home settings (home settings may involve unpleasant hygiene, odor, and some exposure to second-hand smoke).
Physical Requirements
Able to spend time sitting, driving, and negotiating up and down stairways.
Some lifting of office equipment, supplies up to 30 lbs.
Able to listen and communicate verbally.
Mental Requirements
Requires ability to adjust and adapt to changing priorities/work schedules.
Be able to process new information, think, and conceptualize tasks as appropriate.
Able to work with challenging clients.
