Special Service for Groups, Inc.Job Announcement Title : Outreach and Engagement Case Manager Division : HOPICSFLSA : Non-Exempt, Full time Supervisor : Program ManagerPay Range or Rate : $25.00-27.00 per hour Revised : 2.4.2025SummaryUnder the supervision of the Program Supervisor and / or Senior Manager for Mental Health Services and / or the Associate Director, the Incumbent (Outreach & Engagement Case Manager) will serve as the primary representative for the TRC Program during all outreach and engagement events. The staff may also perform professional system navigation, service coordination, and advocacy services for mentally ill and emotionally disturbed consumers who have been victims of or witnesses to a crime. Staff will work with the multi-disciplinary team, assess clients to determine the level of services, and assess additional needs. Staff will assist clients and their significant relational figures in understanding and developing solutions to problems that lead to and result from emotional and mental disturbances. S / he will provide hands-on support to victims of crime and serve as the primary liaison to the Victims-Witness Assistance Program (VWAP), providing trauma-informed care best practices. Staff in this position will need specialized training in treating trauma, collaborating with Law Enforcement, Victims' Advocates, and the District Attorney's Office.Essential FunctionsServe as primary representative, attendee, and / or facilitator for the TRC Program during all outreach and community engagement events.Serve as an advocate for clients, accompany the client to service locations, and serve as the main liaison between our program and law enforcement, VWAP, and outside departments as needed.Communicate with various sources including courts, Los Angeles County Law Enforcement Agencies, District Attorney Offices, Victims Advocate Representatives, Department of Children and Family Services, and other community agencies.Provide technical assistance and hands-on support to help clients access and navigate services.Accompany victims to appointments and court appearances.Develop community resources related to vocational, housing, education, financial, and other areas for children, youth, adults, and families.Possess working knowledge of victims' rights and community-based support groups.Collaborate with assigned specialists; provide telehealth services and / or travel within the community to meet clients and assist with completing necessary paperwork.Attend weekly Multi-Disciplinary Team meetings and be prepared to present case management or rehabilitation-related concerns.May conduct groups and co-facilitate groups in accordance with the designated evidence-based model.May assist with processing new client referrals and client walk-ins as needed.Be responsible for assistance with benefits establishment, with VWAP, and assistance to all clients; must be familiar with mainstream benefits guidelines.Be the primary person responsible for community outreach to increase awareness and inform organizations, hospitals, law enforcement about the Center and develop working relationships with other agencies whose services will be beneficial to clients.Participate in all meetings with law enforcement and District Attorney training.Attend all meetings related to this grant; update Individual Tracking Log regularly.Document effectively in accordance with established standards of care daily.Assist with completion of non-clinical documentation.Adhere to weekly paperwork deadlines and document in accordance with best standards of practice.Assume other tasks as assigned by the Program Manager and / or Senior Manager for Mental Health Services and / or the Associate Director.Participate in all mandatory programs, division meetings, and trainings (including offsite meetings and trainings), as assigned by the Program Director, Senior Manager for Mental Health Programs, and / or the Associate Director.Maintain appropriate boundaries and adhere to SSG’s Code of Ethics and HOPICS’ Core Values.Represent the Agency in a professional manner at meetings and community events; maintain regular attendance.Maintain files / records on client services in compliance with HIPAA, CFR42, and other funding requirements for auditing purposes.Maintain regular attendance and may be assigned other duties as needed.Minimum Qualifications - Knowledge, Skills, and Abilities RequiredBachelor's degree from an accredited college or university in Social Work or a related field with two years of related experience or a master’s degree (MSW or MFT) from an accredited college or university in Social Work.Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.Ability to communicate effectively both written and orally with collaborative entities; neat and professional appearance.Attention to detail and punctuality required; working knowledge of Microsoft Word, Excel, and Internet.Experience working with homeless people with co-occurring issues (mental health, disabilities, substances); able to report legal issues that surface (Tarasoff, child abuse, vulnerable adult abuse, danger to self / others, and grave disability); professional experience working within a multi-integrated team.Coordinate the submission of client documentation if unable to work; if in recovery, a minimum of three (3) years of being drug and alcohol-free is required. MANDATORYVerification of Employment Eligibility and Background Check.Maintain access to reliable transportation supported with active insurance and a valid Driver's License.Work with co-occurring populations that present with any stand-alone or combination of mental illness, health conditions, and substance-related concerns.Knowledge of the medical model and evidence-based practices.Bilingual preferred but not required.Supervisory Responsibilities : This position does not have any supervisory responsibilities.Environmental Conditions (Working Conditions)May be exposed to aggressive / hostile clients; dealing with trauma may trigger personal trauma. There is some responsibility to work in noisy environments where adults are free to talk loudly and expressively. Must be able to work effectively and produce quality work under pressure. While driving to training or performing other duties, there may be exposure to weather conditions, noise, moving vehicles, high crime rate areas, odors, fumes, pollen, etc.Physical RequirementsThe employee is regularly required to sit; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms; and talk or hear; frequently required to stand, walk, and kneel; occasionally climb, balance, or stoop; rarely crouch or crawl.
