While working closely with physicians and therapists, enjoy the independence of a home care setting during your one-on-one shift with a client. RNs and LPNs welcome to apply.
We are seeking individuals that are dependable and dedicated to making a difference and taking an active role in helping others at CC Rehab. Home Care Nurse (Registered Nurse, or Licensed Practical Nurse) provides treatment, medication, diet, rehabilitative care, activities of daily living, protective service, and self-care training under optimal comfort and care conditions. Home Care Nurses provide guidance and direct delivery of clinical nursing care to Home Health Care clients by utilizing appropriate and required nursing interventions in response to the direction of a physician.

Responsibilities
- Provides one to one direct supervision and support to clients in their homes and, or other treatment settings.
- Participates in all pre-admission assessments of potential residential clients.
- May write or assist with treatment plans, progress reports, doctor's letters, and completes performance (outcomes, quality improvement) measurement data forms in a quality and timely manner
- Provides triage and guidance to staff in handling any changes in the medical status of clients.
Qualifications
- Graduation from a state approved school of nursing and holds a current license to practice in the State of Michigan.
- Two years nursing experience preferred.
- Experience working in a home care/residential environment.
- Specialized training or experience in rehabilitation, subacute, long term care, or rehabilitation preferred
