IDD Qualified Professional

Specialized Health Care Services LLC is a provider agency that provides services to individuals with intellectual developmental disabilities. We are seeking a full-time Qualified Professional to join our team. The QP will work closely with the Director to provide support in the administration of the services.

The QP is an experienced professional responsible for implementing, managing and the coordination of the habilitation process for individuals who are intellectually disabled. The QP ensures that quality of care, services, and training are provided to individuals. The QP will be committed to giving each individual the opportunity to lead meaningful lives, be a part of the community, make personal choices and achieve maximum independence. The QP independently manages & coordinates overall development & implementation of client-based habilitative programing (e.g., individual program plan, individual behavior guidelines, etc.):

Responsibilities

• Assist with the coordination of new admissions and transfers (e.g. screening applicants, conducting tours, facilitating placement, etc.)

• Ensure that all direct care staff are qualified and properly trained to work with clients based on their needs.

• Ensure billing information is completed accurately.

• Ensure documentation standards are met.

• Document conversations with parents/guardians, Care Managers and other parties as appropriate.

• Manage staff on a day-to-day basis; address and document performance issues in a timely manner.

Pay: $23.00 - $25.00 per hour

Expected hours: No less than 40 per week
