About the Job
• Developing and implementing comprehensive patient care plans in conjunction with multidisciplinary teams
• Accurately recording patient condition and care, maintaining detailed clinical notes and summaries
• Assisting patients with daily living activities, teaching self-care techniques to promote autonomy
• Maintaining accurate records and adhering to universal precautions and emergency procedures

Requirements
• Approximately 1 year of experience as an LPN in home care, med surg, or ED setting
• Current state license as an LPN
• IV certification
• CPR certification and valid driver's license
