Key Responsibilities:
• Develop and implement comprehensive patient care plans in conjunction with multidisciplinary teams
• Accurately record patient condition and care, maintaining detailed clinical notes and summaries
• Assist patients with daily living activities, teaching self-care techniques to promote autonomy
• Maintain accurate records and adhere to universal precautions and emergency procedures
Requirements
To be successful in this role, you must have:
• Approximately 1 year of experience as an LPN in home care, med surg, or ED setting
• Current state license as an LPN
• IV certification
• CPR certification and valid driver's license
