With eight office locations, hospital affiliations across the region, and partnerships with multiple outpatient dialysis facilities, we ensure our patients receive the highest standard of nephrology care.
About the Role
We are seeking a part-time Nurse Practitioner (NP) to join our growing team. In this role, you will play a vital part in diagnosing and managing kidney-related health conditions while promoting preventive care and patient education. As an advanced practice provider, you will work closely with physicians, patients, and interdisciplinary teams to deliver high-quality, patient-centered care.
The ideal candidate will have experience in internal medicine, nephrology, endocrinology, cardiology, or acute care, with a strong preference for those with nephrology expertise and familiarity with value-based care models.
Key Responsibilities
- Conduct comprehensive patient assessments, including medical history reviews and physical exams
- Order, interpret, and analyze diagnostic tests (e.g., blood tests, imaging, lab results)
- Diagnose and develop treatment plans for patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD)
- Prescribe medications and manage patient treatment regimens
- Provide dialysis education and discuss treatment options with patients and their families
- Perform outpatient ESRD management and dialysis rounds during the daytime
- Maintain accurate patient records and schedules
- Educate patients on preventive care and long-term kidney health
- Stay informed on the latest CKD/ESRD research and treatment advancements
What We’re Looking For
- Licensed Nurse Practitioner (NP) with an active, unrestricted license
- Experience in nephrology, internal medicine, cardiology, endocrinology, or acute care preferred
- Strong familiarity with value-based care models is a plus
- Proficiency in ARF (Acute Renal Failure) and CRF (Chronic Renal Failure) management in an outpatient setting
- Strong understanding of HIPAA regulations and patient care guidelines
- Excellent communication, problem-solving, and critical-thinking skills
- A team player with a patient-centered mindset and a positive attitude
- A commitment to ongoing professional development and evidence-based practice
