Case Manager -Stepping Up Program (Barbour County)
This position will be based in Eufaula, AL
Working hours for this position will be Monday - Friday 8:00am - 5:00pm
POSITION SUMMARY/FUNCTION
Responsible for providing case management services to seriously mentally ill and/or substance use adults who are currently or have been previously hospitalized and/or in the jail systems. Position will be responsible for completing and implementing case management plans with the goal of preventing future hospitalizations and/or incarcerations through linking recipients with community resources and continuing behavioral health care. Will be responsible for maintaining appropriate records. Some travel may be required within the catchment area.
ESSENTIAL DUTIES/RESPONSIBLITIES/FUNCTIONS/TASKS:
1. Coordinates and provides case management services to recipients in the “Stepping Up” program.
A. Complete case management needs assessments and develop a case management plan;
B. Implement plan by providing case management services in conjunction with county jails and area hospital staff;
C. Advocate recipient’s needs with community providers;
D. Link recipients with community services;
E. Monitor progress of recipients in the program.
2. Represents the needs of the persons admitted into the “Stepping Up” program and insures quality services are provided.
A. Participates in and conducts “Stepping Up” related trainings.
B. Complete work in accordance with SpectraCare and “Stepping Up” standards;
C. Participates in meetings of the “Stepping Up” Alliance Task Force.
D. Participates in SpectraCare, county jail, and hospital staff meetings as required.
3. Completes needs assessments as required.
A. Using required case management forms, determines through assessment recipient’s assets, deficits and basic needs; reviews needs on a regular basis.
4. Develops and implements case management service plan for recipients.
A. Consult with referral source(s);
B. Assist with developing client goals which may include but not limited to personal grooming, skills teaching, medication knowledge, and financial resource assistance, etc.
C. Coordinate service delivery to recipients among appropriate agencies as specified in the service plan.
D. Provide family support and/or basic living skills as needed.
E. Conduct periodic assessment and modification of service plan.
F. Advocate on behalf of recipient and target population as a whole regarding social, and/or economic requirements.
G. Conduct home visits.
H. Provide transportation only when it is a necessity. Ensure all community resources such as family, friends, public transportation and special vehicles are utilized to meet the needs of recipient. If transporting clients in personal vehicle, must maintain additional liability insurance as required.
I. Coordinate care for recipients between agencies and monitor/follow up with the recipient and agency regarding this care.
J. Close cases as appropriate.
K. Meet required productivity level of direct service time.
5. Represents the needs of persons with serious mental illness and co-occurring substance use disorder issues.
A. Establish liaison with other community resources;
B. Identify service gaps;
C. Advocate for the individual;
D. Provide transportation as needed and when appropriate.
6. Assist with maintaining recipient records in a professional manner and in accordance with all SpectraCare policies and procedures, Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities.
A. Ensure privacy and confidentiality of all recipient records in accordance with HIPAA, 42 CFR and all other applicable state or federal laws.
B. Ensure all written documentation to include but not limited to SUN-R assessments, individualized service plans, etc is completed accurately and in a timely manner.
C. Ensure that all records are staffed within standard time frames, with service plan revised as needed and in accordance with regulatory requirements.
D. Ensure that all progress notes are completed promptly, written in a professional manner and in accordance with policies and procedures.
E. Ensure that goals are appropriate, specific, and the focus of treatment.
F. Ensure that all recipient information is kept up to date.
G. Ensure recipient release of information forms are signed in a timely manner and maintained within the chart.

G. Receive and respond to inquiries promptly and courteously.

K. Maintain travel records accurately and submitted within the appropriate timeframe.
L. Ensure necessary documentation is completed in a timely manner with minimal errors and in compliance with applicable guidelines and policies.
M. Adhere to Dress Code as set forth in the SpectraCare policies and procedures.
N. Follow guidelines of SpectraCare Personnel Policies, SpectraCare Clinical Policies, Alabama Department of Mental Health Administrative Code, CMS, and all other applicable regulating and contractual entities.
O. Performs duties independently and/or with minimal supervision.
P. Functions within professional boundaries with recipients and others.
Q. Works well with other staff and recipients to ensure safety of program, recipients, and other staff.
R. Functions and performs well in stressful environments and manages work related stress to ensure individual safety.
S. Demonstrates willingness to work on any unit / program at SpectraCare based on facility needs as directed.
T. Other services, tasks, functions, and responsibilities deemed necessary; not otherwise specified.
9. Fulfill duties of Nurse Delegation Program.
A. Become fully certified as a Medication Administration Certified (MAC) assistant.
B. Follow all regulations as outlined in the MAC policies including but not limited to the
Seven (7) Rights and Responsibilities, training, consultation with RN/LPN, error reporting,
etc.
C. Comply with all documentation requirements of the MAC program including the MARS,
counting and recording controlled substances, PRN medications, sign out sheets for home visits, and medication errors reporting sheets.
10. Regular attendance with timely arrival and departure according to scheduled hours.
a. Must be able to work full time with rotating assignments as necessary to include but not limited to overtime hours, nights, and/or weekends as scheduled.
b. To ensure recipient/employee safety while on duty, must remain awake during scheduled working hours.
c. Comply with SpectraCare’s attendance policy.

ESSENTIAL QUALIFICATIONS/KNOWLEDGE/SKILLS/ABILITIES:
- Knowledge of psychiatric and substance use disorders in the adult population and/or knowledge of addiction as a primary/secondary illness.
- Knowledge of cultural diversity
- Knowledge of behavior management, crisis intervention, and interventions common to acute psychotic individuals and/or individuals with substance use issues as well as , to non-violent crisis intervention practice.
- Knowledge of available community resources.
- Knowledge of educational methods appropriate to adults with sever and persistent psychiatric disorders
- Ability to design, implement/administer individualized behavioral services in regard to case management services.
- Ability to provide services in all facilities and other locations as deemed necessary.

ESSENTIAL TRAINING REQUIREMENTS:
- Complete initial new hire training.
- Complete required on the job training (OJT) training by the department.
- Obtain proper certifications needed to perform job duties.
- Complete annual training as required by the Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities.
- Complete Care Coordination / Case Management Training as required by the Alabama Department of Mental Health
- Complete First Aid/CPR upon hire and working alone with clients; and continuously thereafter maintaining current certification.
WORK ENVIRONNMENT / PHYSICAL REQUIREMENTS:
- Physical ability to work assigned shift. May require periodic weekend/evening work to meet the organizations’ needs.
- Must be able to travel to system facilities and/or other locations/outings with exposure to changing weather conditions.
- Work location is primarily in a temperature-controlled setting with minimal periods of sitting at a desk with associated repetitive motions. Must understand and practice good ergonomic movements to avoid injuries.
- Physical ability to correctly lift and/or moves up to fifty (50) pounds on an infrequent basis.
- Required to talk to or hear staff, individuals served, and/or others.
- Required to stand, walk, sit, bend, twist, and/or reach frequently.
- Employee may be exposed to one or more of the following: disagreeable odors, chemicals such as disinfectants, cleansers, soaps, etc; latex, plastic and other materials used for personal protective equipment; noise and distractions; unpleasant sights; unpredictable behaviors; contagious diseases; and/or wet floors.
- Required to train professional staff through verbal and visual presentations.

REQUIRED QUALIFICATIONS/EDUCATION/EXPERIENCE:
- Bachelor’s degree in psychology, social work, human services and/or related field.
- Must have a valid driver’s license with clean MVR.
- Must maintain automobile liability insurance.
- Willing to complete Case Management training upon hire.
PREFERRED QUALIFICATIONS/EDUCATION/EXPERIENCE:
- Prior case management experience in a mental health/substance use setting.
