Job Description :
We are seeking a compassionate and experienced Long Term Care Manager to manage a caseload of healthcare members with long-term care needs in the Pinellas County area. This is a remote role with 80% of the work being member-facing, involving travel to members' homes or facilities. As part of your responsibilities, you will :
- Conduct monthly and quarterly member contact, including in-person visits and telephonic follow-ups.
- Complete assessments with members, caregivers, and providers to gather information on client status, support systems, and service needs for care plan development.
- Monitor the delivery of services, ensuring follow-up with members, caregivers, or providers to avoid gaps in care.
- Authorize and coordinate referrals for services to meet the members’ needs.
- Collaborate with discharge planners, physicians, and other stakeholders to ensure proper care coordination and seamless discharge and care planning.
- Assist members with filing complaints and appeals to resolve issues efficiently.
