Job Title: Long-Term Service Care Manager
Location: Remote with Member Facing - Amarillo/Canyon, TX
Duration: 6 Months (Contract to hire)
Schedule: Mon-Fri, 8 am-5 pm
Responsibilities:
- Conduct 3 days of member visits and 2 days of documentation weekly.
- Manage a caseload and complete assigned tasks.
- Perform H2060 assessments and complete required forms.
- Make follow-up calls and document within a 3-day turnaround.
- Maintain organization to ensure timely completion of tasks.
- Achieve a 90% or higher in audit scores.
Requirements:
- Experience in Social Services, Social Work, Care Management, or Service Coordination.
- Nursing/LVN/Medical background preferred, with at least a bachelor's for social work or a license for LVN.
Education:
- Bachelor's Degree required
