Job Title : LTSS Service Care Manager
Location : Remote with Member Facing in Sarasota / Collier / Lee, (County) Florida
Mon-Fri 8A-5P
Day-to-Day Responsibilities :
- Manage a case load of healthcare members with long-term care needs.
- Monthly and quarterly member contact, including 80% travel. Remote role requiring a driver’s license.
- Complete member assessments with caregivers or providers to gather information for care plan development.
- Monitor service delivery, ensuring that services are provided without gaps. Follow-up with members, caregivers, or providers through in-person visits and telephonic contact.
- Authorize and coordinate service referrals, ensuring functional deficiencies are identified and addressed in care plans.
- Assist in coordinating the development of informal or voluntary services within the member care plan.
- Collaborate with discharge planners, physicians, and other parties to ensure a proper discharge plan, care plan, and coordination of services.
- Assist members in filing and resolving complaints and appeals.
