Position Overview:
The Support Coordinator will play a key role in our care management team, providing crucial support to individuals with developmental disabilities. This is a case management position that requires a proactive, detail-oriented individual who excels in communication and organization.
As the Support Coordinator, you’ll work collaboratively to develop person-centered care plans, connecting clients and their families to resources and services that best meet their unique needs and preferences. You’ll conduct ongoing monitoring of the provision of services and provide quarterly in-person visits, with monthly check-ins via phone.
This position requires flexibility to accommodate client needs.
*This role is located in Salem/Cape May/Cumberland County.*
Key Responsibilities:
- Develop person-centered care plans tailored to each client’s needs
- Help clients and families navigate and access community resources for adults with developmental disabilities
- Conduct home visits at least quarterly and maintain monthly phone contact
- Provide continuous oversight and monitoring of service delivery to ensure quality care
- Maintain detailed case notes and documentation
- Collaborate with the care team to address any changes in service needs or plans
- Adhere to all state and organizational guidelines for care management
Job Requirements:
- Bachelor’s Degree or higher in Social Work or a related field
- Minimum of 1 year of case management experience and 1 year of experience working with individuals with developmental disabilities
- Experience can include paid employment, volunteer work, or caregiving
- Experience with care coordination for other populations, where some individuals had developmental disabilities, can also be considered
- Valid New Jersey driver’s license, reliable vehicle, and willingness to travel
- Ability to work independently, with strong organizational skills and attention to detail
- Proficiency in Microsoft Office, Adobe Acrobat Pro, and ability to operate office equipment
- Home office setup is advised , including access to a laptop, printer, cell phone, WiFi, and a hotspot

*This position is on a per case basis. The caseload for this position varies by location, as it is determined by the number of clients in your area. This role operates on a per-case basis, and you will receive compensation for each case you handle. You will have a minimum caseload of 15 clients.*
