SPECIFIC DUTIES AND RESPONSIBILITIES:
Administrative
- Coordinates and assists in the development and implementation of the Individual Support Plans (ISP) and to documentation of progress
- To support individuals in their growth towards greater independence in self-care, daily living skills, socialization and community living
- Responsible for ensuring all services are documented in accordance with regulations and agency policies and procedures
- Reviews daily notes to ensure staff compliance with outcomes
- Completes monthly and quarterly reports on each individual assigned to the caseload
- Attends professional development sessions monthly and maintains required certifications in compliance with the applicable State Regulations
- To support, supervise and provide assistance to programs in reference to goal and plan implementation and training in programmatic issues
- Ensures waiver renewal with the Medical Assistance Office
- Ensures AIS/SH/HCH is applied for as needed and that renewal applications are reviewed by the team and completed one month prior to the expiration of the service
- Performs work assignments with minimum need for supervision and guidance
- Originates actions and actively influences events. Offers suggestions without waiting to be asked
- Demonstrates ability to set goals, assign priorities, anticipate problems, establish limits, and achieve result
- Participates in the development and implementation of Individual Service Plan (ISP)
- Completes all intakes of new individuals
- Prepares for, schedules, conducts, and attends all quarterly meetings
- Attends and participates in all annual ISP meetings
- Develops assessment and updates Lifetime Medical History and protocols
- Ensures all residents receive required and necessary medical/dental/psychiatric/all other care/appointments in a timely manner and ensure follow-up occurs on all recommendations by the individual’s health care provider
- Participates in Systems Intensity Scale (SIS) assessments when conducted and ensures informants are present who have knowledge of the individual’s skill levels
- Participate in Consumer Data Collection (CDC) when conducted and ensures all medical records are available for review, including archived records if necessary
- Provides overall case management and advocacy for all individuals
- Assists the individuals in the expression and fulfillment of their needs through the development of a community life plan, its schedules and updates for each individual
- Understands the purpose of adaptive equipment; makes recommendations as to adaptive equipment that would allow individual to be more independent and functional; insures adaptive equipment is available and used by the individual.

- Conducts and participates as a team member in supervisor, staff and team meetings
- Completes files and maintains all required documentation which includes monthly summaries, quarterly and annual ISP reports, incident reports, and other related forms as directed by supervisor
- Acts as HCSIS Point person and enters and investigates incident reports
- Updates all demographic and medical emergency information in individuals’ records
- Required to complete and pass the Certified Investigator’s training and complete and pass the renewal process every 3 years.
- Participate as an active member of the monthly Incident Management Review Team and ensure completion of incident documentation according to Incident Management guidelines.
- Reports to work, meetings and appointments in a punctual mannerprepared to work.
- Keeps supervisor well informed of status of assignments.
- Responds to requests and leadership of supervisor
- Helps supervisor get support for departmental goals and plans
- Supports and follows all agency policies and procedures as well as complying with all applicable State regulations
- Ensures that the programs, vehicles, and equipment are safe, functional, and secure for the individual’s physical well being
- Manages relationships with individuals, families, team members, staff, advocates and neighbors by responding to inquiries with tact and sensitivity.
