SUMMARY OF POSITION FUNCTION:
Knowledge and demonstration of Agency Core Values in all day-to-day activities. Provides medical care to the Agency’s behavioral health programs. Duties are in keeping with scope of practice which include interviewing clients to screen for medical problems, health monitoring, injections and education, along with coordinating referrals for follow-up with medical offices. Duties also include individual and group services regarding the relationship between health and behavioral health issues. Special projects as assigned to include gathering data, tracking and monitoring of individuals who are high risk. Oversite and training of new employees. Provide leadership to the other LPN’s on site.
MAJOR DUTIES AND RESPONSIBILITIES:
- Knowledge and demonstration of agency core values in day to day activities
- Meets documentation timeliness standards
- Demonstrates ability to effectively use technology and appropriate use of support
- Follows Agency policies and procedures
- Performs general professional nursing duties including medical observation and gathering relevant medical data and history, injections, gathering and review of labs and medical reports received, pharmacy questions, etc. Reviews with RN, psychiatrist/NP and pharmacy/lab as necessary and responds accordingly
- Responds to all site medical crises including first aid, CPR, AED, etc. On-site and recommends direction for next level of care (i.e. – call to PMD, ambulance, etc.)
- Appropriate documentation
- Continually attends to and provides for Evidence based needs including metabolic, cardiac, EPS, nicotine, drug, alcohol, and trauma
- Responsible for maintaining needles, medications and medical waste/waste pickup in accordance with Agency policy
- Administers IMs as ordered and follows appropriate protocol.
- Assists prescribers with Dr. Homepage including updating of medications and medical information.
- Works with the nursing team, pharmacies and clients to resolve medication issues
- Serves as resource person/consultant to other service providers in identifying clients for referral and works cooperatively with other providers to assure linkage of clients with appropriate services.
- Assures that advocacy, linking, expediting and related activities are carried out as needed
- Proficient in the use of Agency software such as Microsoft Office Products and Cerner.
- Performs other duties as assigned.
- Provide leadership to the other LPN’s on site.

SKILLS/COMPETENCIES:
- Qualified to provide medical services such as conduct medical history review, obtaining basic vital signs and providing IM’s to clients
- Skilled in providing education regarding medical/medication issues.
- Skilled in use of Cerner/excel
- Demonstrated skill in relating to populations served with respect, accuracy and timeliness.
EDUCATION REQUIREMENTS:
- Minimum requirement of LPN education
- Educational qualifications must match regulatory requirements to perform medical services
- Must be licensed practical nurse
EXPERIENCE:
- Experience must match regulatory requirements
- Must have experience in providing medical services to individuals with behavioral health needs
- Minimum one year experience is LPN
- Must possess a valid Driver’s License with an acceptable driving record.
COMPENSATION: $23.09/hr - $29.44/hr
