We are seeking a Licensed Practical Nurse to join our team! You will be responsible for delivering high quality care to assigned patients.
Responsibilities:
- Care for ill, injured, or convalescing patients
- Provide basic patient care and treatment
- Collaborate with registered nurses to administer prescribed medications
- Sterilize and prepare medical tools and equipment
- Educate and update patients on medical treatments
Essential Functions
• Obtains and records patient’s vital signs and weight
• Assists Provider with patient examinations, perform procedures and wound care, as needed
• Administers specified medication, orally or by subcutaneous or intermuscular injection, and notes time and amount in patients’ charts
• Collects samples, such as urine, blood, and sputum, from patients for testing and performs routine laboratory tests on samples
• Reviews Provider’s orders, lab requests, or follow-up needs with patients
• Interviews patient to determine medical problem/condition and documents in chart for physician
• Assist patients with scheduling follow appointments
• Complete and submit prior authorizations for imaging, medication, procedures
• Processes patient referral requests
• Completing task in assigned buckets (assigned site, providers, nurse, personal, etc.) regarding patient question/problems, medication refill request, telephone inquiries from affiliated agencies and others
• Communicate with affiliated agencies regarding test results and updates on patients
• Supports Chronic Care Management Team
• Assist with stocking and ordering new supplies and disposing of expired or damaged supplies
• Assist in administering Covid-19 testing and education
• Sterilizes equipment and supplies
• Act as back up for patient flow
• Meeting with pharmaceutical representative and receiving medications and aiding with connecting with providers
• Oversees Vaccine for Children (VFC) Program
• Maintains medical equipment and monitors need of equipment.
• Assist with onboarding on new CMAs
• Participates in community education programs or health programs on and off-site
• Provides education to patients as needed
• Performs other duties as assigned
Requirements
Minimum Qualifications:
- At least One year certificate from college or technical school for Licensed Graduate Practical Nursing; At least One year of practical experience in a community health center or an allied health field environment.
