Traveling LPN Wound Care Nurse
Location: Multiple Facilities
Job Type: Full Time/Part Time/PRN
Job Summary:
we are seeking a dedicated and skilled Traveling LPN Wound Care Nurse to provide specialized wound care services across multiple health care settings, including long-term care facilities, skilled nursing, assisted living, LTACH, and inpatient rehabilitation hospitals.The Ideal candidate will conduct full facility skin sweeps, perform dressing changes as needed and collaborate with the facility and the wound care providers during and after rounds to ensure optimal wound healing and patient care.
Key Responsibilities:
1. Perform head to toe skin assessments for patients and residents across assigned facilities
2. Provide wound treatments based on provider orders, evidence-based practice guidelines, and facility policies and procedures.
3. Assist the assigned facilities as needed to conduct dressing changes and ensure proper wound management to prevent infection and complications.
4. Document skin sweeps on skin assessment form for the assigned RN to review and scribe during provider rounds as needed.
5. Educate facility staff on wound prevention, treatment protocols, and best practices in skin integrity management.
6. Travel between multiple facilities as assigned, maintaining flexibility in scheduling and patient care needs.
1. Active LPN license in the state of Oklahoma
2. Experience in wound care
3. Strong assessment, critical thinking, and documentation skills.
4. Ability to work independently while collaborating with healthcare teams.
5. Reliable transportation for travel between facilities
