Position Summary
The Case Manager - Children's Behavioral Health coordinates treatment activity, participates in client staffing, implements treatment plans, provides rehabilitation services, provides general healthcare management activities, and maintains client records. As healthcare reform advances and health homes become the norm, the Case Manager - Children's Behavioral Health is required to expand their capacity in healthcare navigation, build on existing skills in behavior health change, and gain a better understanding of the common health problems and basic interventions for individuals with mental illness and those with chronic health problems.
Case Manager - Children's Behavioral Health interacts with clients and staff in a culturally competent manner that takes into account factors such as age, developmental or acquired disabilities, religion and spiritual orientation, ethnicity, socioeconomic status, sexual orientation, indigenous heritage, national origin, gender, and gender identity. Enhances professional growth and keeps informed of evidence-based practices in behavioral health field related to populations served and modalities used to include working knowledge of DSM V, and its criteria for mental health disorders. Follows policies and procedures and performs other duties as assigned by supervisor.
Case Manager - Children's Behavioral Health assists with managing internal requests for providers and assists with coordination of high-needs cases among the department providers. Assists and coordinates with hospital discharges within the department and is responsible for providing a high level of crisis assessments and intervention in community settings.

High Needs Case Managers:

  • Provides High Needs Case Management services to children enrolled in the behavioral-health system who meet the criteria for high needs.
  • Provides weekly contact with assigned clients, which includes home/community visits, attending medication checks, etc.
  • Assists youth in navigating admission and then discharge from hospitals or any other out of home treatment placement, in finding intensive services as clinically indicated, and working to decrease behaviors to improve stability in the home & community.
  • Coordinates services and collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
  • Coordinates and facilitates Child & Family Team (CFT) Meetings.
  • Completes referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services.
  • Collaborates with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner.
  • Works with clinical team to transfer clients to the appropriate level of case management for that client.
  • The Case Manager - Children's Behavioral Health is a full-time, non-exempt, hourly position typically working Monday through Friday business hours (some flexibility may be offered) and works on-site and in the community. Local travel will be required and company vehicles will be provided and/or mileage will be reimbursed.

Required Qualifications:

Care Managers:
I: Bachelor's degree in any field and two years experience working with children OR High school diploma or GED plus any combination of behavioral health education, training and experience totaling four years;
II: Bachelor's degree in a behavioral health field and 1 years behavioral health work experience.
III: Master's Degree in a behavioral health field OR Master's degree in any field with 30 semester hours of behavioral health courses and 1 years of behavioral health work experience.

High Needs Care Managers: two years as a care manager and a bachelor's degree in a human services-related field, or an equivalent combination of experience and education
