What are the responsibilities and job description for the Momentum Care Manager, Residential - Cottonwood position at Spectrum Healthcare?
Responsibilities:
The Care Manager for Momentum, our residential programs, reports directly to the Operations Manager. Works with multidisciplinary team to ensure client's needs are met; facilitates referral process for incoming clients to ensure program remains at full capacity; coordinates care with outpatient providers;
works with the client and care team to develop discharge plan; follows up with clients post discharge; and provides direct client care as needed including supervision, medication, groups, transports, outings, etc.
Qualifications:
I: Bachelor's degree in any field and two years of experience working with population (SMI or children, as appropriate) OR High school diploma or GED plus any combination of behavioral health education, training and experience totaling four years, or;
II: Bachelor's degree in a behavioral health field and 1 years behavioral health work experience, or;
III: Master's Degree in a behavioral health field OR Master's degree in any field with 30 semester hours of behavioral health courses and 1 years of behavioral health work experience.
