Job Summary
The duties of a LMHC or LSW include, but are not limited to, commute to multiple post-acute care facilities every day, making rounds, communicating to patients, facility staff members, providing accurate assessment, diagnosis, treatment, consultation, education and follow-up care to patients. Work with Psych division providers within the post-acute facilities. MUST HAVE A FLORIDA LICENSE OR BE A REGISTERED INTERN IN FLORIDA.

Primary Functions
Reports to the Supervisory Psychiatric Physician / Executive Director
Physical Requirements

Primary Functions
- Examine patients and record history
- Adhere to proper charting protocols
- Discuss with Physician and other health care professionals to prepare comprehensive plan
- Submit health care plan and goals of individual patients for periodic review by Physician
- Promote health by advising patient on diet, hygiene and prevention of disease
- Refer to specialist as required for patient's well-being
- Report deaths and contagious diseases to governmental authorities
- Other responsibilities as assigned
- Must have and maintain current State of Florida license in good standing
- Must be LMHC/LSW in the State of Florida
- Current National Board Certification
- Effective written, verbal and electronic communication skills
- Show initiative, creativity and problem-solving ability
- Ability to collaborate with colleagues to provide quality healthcare
- Prior experience using an electronic medical records system
- Bilingual a plus, but not required (English/Spanish)
